Waiver & Release Agreement

This liability statement allows guided hunting and related activities in the state of Ohio for the purpose of waterfowl hunting with Crooked River Guide Service and/or its representatives. Crooked River Guide Service does not make any warranty or representation of any type, kind or character, whatsoever, as to existing conditions upon said lands and equipment for the purposes of waterfowl hunting or related activities.

The undersigned acknowledges and agrees that the hunting related activities may involve the risk of property damage, personal injury and or death. John Beckwith, Brian Laux,  their agents, other hunting guests and/or employees of Crooked River Guide Service do not and can not undertake to supervise, control and/or regulate any hunting or related activities.

I, myself, or as parent or guardian of any accompanying minor, unconditionally release and hold harmless John Beckwith, Brian Laux and Crooked River Guide Service or it's representatives against and for all liability, cost, expenses, claims and damages for which Crooked River Guide Service might otherwise become liable by reason of any accident, illness or injuries to or death of any persons, or damage to property or both, in any manner arising or resulting from, caused by, connected with or related to the participation in any activity or service offered by Crooked River Guide Service, regardless of how, where, or when such injury, illness, death or damage occurs even if caused by the negligence of Crooked River Guide Service, its agents, servants, or employees, or due to conditions, participation or defects in the premises, equipment or activities.

For and in consideration of engaging in waterfowl hunting and related activities with employees and/or agents of Crooked River Guide Service, the undersigned waives and releases any claims, actions or damages which he/she my incur, directly or indirectly, and forever discharges John Beckwith, Brian Laux and Crooked River Guide Service as well as their families, employees, agents, successors, personal representatives and assigns from any liability or responsibility, of any kind, with respect to said claims, action or damages.

I agree and acknowledge that my hunting related activities are the result of my voluntary acts and decisions. I am over the age of 18 years and I have read and fully understand the contents of the Waiver/Release Agreement. This agreement is a voluntary expression by me of my knowledge and intent. I have signed this Waiver/Release Agreement as my free and voluntary act and I intend that this Waiver/Release constitutes a contact rather than a mere recital.

I do certify that I have all the necessary licenses and/or permits to participate in any of the aforementioned activities with Crooked River Guide Service, and that I do not hold Crooked River Guide Service responsible for any lack thereof licenses or permits.

To the best of my knowledge I have no physical restrictions which would prohibit my participation in any and all activities offered by Crooked River Guide Service, including hunting, hiking, or any other outdoors related to waterfowl hunting by Crooked River Guide Service or it's representatives.

I, the Permittee, have read this release and understand all its term. I execute it voluntarily with full knowledge of its significance.
